Decide how you want to reach Altus Plateau later in the game: A) With the two halves of the Dectus medallion, B) Through the Ruin-Strewn Precipice, accessible going north past the Bellum church, north of the path toward the Dectus Lift, C) By letting the Abductor Virgin at the bottom of Raya Lucaria telport you to Volcano Manor, and defeating the Abductor Virgin duo. (option A is the easiest one, option C is possibly the fastest one)

If you choose to access Altus Plateau with the Dectus Medallion, you need to grab both halves of the Dectus Medallion

Get the Dectus Medallion (Left) from Fort Haight in Limgrave (east Limgrave, south of Mistwood)

Easiest way to reach there is from waygate portal behind Third Church of Marika and going south from Bestial Sanctum, or you can just ride all the way there from Limgrave. To get the item you can just run through all the enemies up the stairs, you don't need to fight anything. Optionally you can get the Radagon's Soreseal in Fort Faroth, which is a good early game Talisman

Approach Murkwater Cave, to be invaded by Bloody Finger Nerijus. Murkwater Cave is found north of Agheel Lake, up the river

After defeating Bloody Finger Nerijus, talk to Yura right north from where Nerijus invaded

Meet Patches at Murkwater Cave as a boss, and accept his surrender (in Limgrave. halfway through the Murkwater River)

Optionally, attack Patches 3 times, then allow him to reduce your HP to 1/4, then use the Grovel for Mercy gesture

Talk to Patches, reload the game and talk to him again to enable his shop. Optionally buy Margit's Shackle

After defeating Godrick, talk to Rogier in Roundtable Hold to get Rogier's Rapier +8 with Ash of War: Glintblade Phalanx

In the Ulcerated Tree Spirit area below Stormveil Castle, find Rogier's bloodstain and activate it, then talk to Rogier in Roundtable Hold

Most of Fia's questline can be skipped. There are two places where Fia's quest is skipped: 1) Activaitng the Radahn Festival: Fia gives you the Weathered Dagger, 2) Burning the Erdtree: Fia will move to Fia's Champions arena in Deeproot Depths

Given as a reward for completing Roderika's Task at Stormhill Shack. Limgrave. If you skipped her quest, rest in any grace in Liurnia and you can get the Golden Seed from where she was in Stormhill Shack regardless.

Give Shabriri Grape to Hyetta in the Lake-Facing Cliffs site of grace in Liurnia

After defeating Godrick and resting at his grace, talk to Nepheli in Roundtable Hold (you might need talk first to Gideon and rest in a grace for Nepheli to appear)

Obtain any of the Great Runes (a good choice is to get Godrick's Great Rune as it's useful and easy to get, or beating Rennala will also get you her Great Rune)

Head north west from the pavilion where Rya is located to reach the Boilprawn Shack, speak to Boggart and buy the necklace from him for 1000 runes (or kill him, if you don't care about his questline)

Buy prawns from Boggart and talk to him. If you plan to follow Dung Eater's questline, but you want Boggart to survive, you can postpone this step until you have completed Dung Eater's questline, to prevent Boggart from moving to Altus. Note that this will prevent you from getting one Seedbed Curse

From Liurnia Lake Shore grace, looking north hug the left wall until finding the entrance to Lakeside Crystal Cave. Progress through the dungeon to find Latenna at the end and grab the grace

Find Hyetta north of the Gate Town Bridge, easter Liurnia of the Lakes (in front of the southern broken bridge in the east side of the lake, above the cliffs) and give her a Shabriri Grape.
